Bioidentical Hormone Therapy: What Patients Should Know

Bioidentical describes hormones with the same chemical structure as those your body makes, but it is not a quality or safety label. Medical societies say FDA-approved options are generally preferred over custom-compounded products.

Bioidentical hormone therapy is a term used for hormones with the same chemical structure as those your body produces, such as estradiol, progesterone, and testosterone. The important distinction for patients: some bioidentical hormones are FDA-approved products, while many products marketed as bioidentical are custom-compounded and have never been FDA-reviewed for safety, efficacy, purity, or potency. Major medical societies, including the Endocrine Society and ACOG, state there is little evidence that compounded bioidentical hormones are safer or more effective than FDA-approved options. At Zest Regen Health in Columbia, Maryland, hormone care starts with testing and a physician evaluation, not with a product name.

What bioidentical actually means

The word bioidentical means the molecule has the same chemical structure as the hormone your body makes. Estradiol, estriol, progesterone, testosterone, and DHEA can all be made in bioidentical form. What the word does not mean is that the product has been tested, approved, or proven safer than other options. It is a chemistry description, not a regulatory or quality category.

FDA-approved versus custom-compounded

FDA-approved bioidentical products exist and include estradiol patches, gels, and tablets, micronized progesterone capsules, and testosterone gels, patches, and standard injections. Custom-compounded products, such as subcutaneous pellets, creams, troches, and sublingual drops, are mixed by a pharmacy for an individual patient and are not FDA-reviewed for safety, efficacy, purity, or potency. When a product has not been through FDA review, its dosing consistency and quality can vary, and there is no clinical-outcomes evidence behind it.

What medical societies say

  • The Endocrine Society (2019 position statement): little or no scientific evidence that compounded bioidentical hormone therapy is safer or more effective than FDA-approved therapy
  • ACOG (2023 clinical consensus): bioidentical hormone is a marketing term, and FDA-approved formulations are preferred when available
  • The National Academies of Sciences, Engineering, and Medicine (2020): insufficient evidence for custom-compounded bioidentical hormones, with widespread use flagged as a public health concern

What testing comes first

Hormone therapy should never be chosen from a menu. The right starting point is a medical evaluation with blood testing that measures the hormones relevant to your symptoms and goals, reviewed alongside your history and risk factors. Testing tells you what is actually out of range, which determines whether therapy is appropriate and which approach makes sense.

Questions to ask before starting hormone therapy

  • Is this product FDA-approved, or is it custom-compounded?
  • What testing supports this treatment plan?
  • What are the potential risks, and how will I be monitored?
  • How will we measure whether the treatment is working?
