GLP-1 Medications and Bone Health: What Patients Should Know
Substantial weight loss on GLP-1 medications can include measurable bone density loss at the hip and spine. Here is what the research shows and how supervised programs plan for it.

GLP-1 medications produce substantial weight loss, and part of that weight can come from bone: the most direct randomized evidence shows measurable bone mineral density loss at the hip and lumbar spine after a year of semaglutide treatment in older adults at higher fracture risk. The losses, roughly 2 to 3 percent at those sites, are small but real and are most pronounced in people who lose the most weight or already have bone risk. At Zest Regen Health in Columbia, Maryland, bone health is discussed before starting a GLP-1 program, not discovered later.
What the most direct study found
A 2024 randomized trial published in eClinicalMedicine followed 64 adults at increased fracture risk for 52 weeks on once-weekly semaglutide 1.0 mg or placebo. Participants on semaglutide lost about 9.4 percent of their body weight and showed significantly lower bone mineral density at the total hip, about 2.6 percent less than the placebo group, and at the lumbar spine, about 2.1 percent less. Bone resorption markers rose while formation markers did not, a pattern researchers describe as an uncoupling of the normal bone remodeling balance.
Why rapid weight loss affects bone
Bone adapts to the load placed on it, so weight loss removes some of that everyday load, and calorie restriction also shifts hormones that regulate bone turnover. The same pattern of bone loss is seen with aggressive dieting and bariatric surgery. The more weight that comes off quickly, the more attention bone deserves, especially in older adults and in anyone with risk factors such as prior fracture, small frame, or a family history of osteoporosis.
What the tirzepatide data show
A body-scan substudy of the SURMOUNT-1 trial, in which participants lost about 21 percent of their body weight on tirzepatide over 72 weeks, found about 3.1 percent lower total hip bone density and 2.6 percent lower femoral neck bone density, with lean mass down 10.9 percent compared with 2.6 percent on placebo. Larger weight loss, larger bone changes. This does not mean the medications are unsafe, but it does mean they deserve planning.
What protects bone during GLP-1 treatment
- Resistance training and weight-bearing activity, which put load back on the skeleton and preserve lean mass
- Adequate protein and overall nutrition, consistent with current obesity medicine guidance
- Vitamin D and calcium status checked and corrected when needed
- Baseline assessment of fracture risk when appropriate, including DEXA scanning for higher-risk adults
A 2024 secondary analysis in JAMA Network Open compared exercise alone, a GLP-1 medication alone, and the combination after weight loss. The combination preserved bone best, and exercise alone outperformed the medication alone at several sites, reinforcing that body composition protection has to be built into the plan rather than hoped for.
A note on the broader body of evidence
Meta-analyses of GLP-1 use in type 2 diabetes are mixed to neutral on bone density overall, and the major weight loss trials found no significant difference in fracture rates over about 18 months, though those trials were not designed to detect fractures. In other words, bone loss is not automatic or universal. It is a risk that is highest with rapid, large weight loss in higher-risk patients, and it is manageable when it is part of the conversation from the start.
