Hormone Optimization Blood Testing: What a Medical Evaluation Measures
A hormone optimization evaluation starts with symptoms plus lab work, and the correct panel depends on your sex and history. Here is what is commonly measured for men and women, and what the Endocrine Society guidance says about testosterone therapy.

Hormone optimization is not a package: it is a medical evaluation of symptoms and lab values that leads to an individualized plan. For men, the most important first step is confirming symptoms of testosterone deficiency together with consistently low fasting morning testosterone on two separate occasions, per Endocrine Society guidance. For women, the relevant hormones depend on age, symptoms, and reproductive stage, and thyroid and other markers are often checked alongside them.
What is typically measured for men?
For men, a standard evaluation includes total testosterone (ideally a fasting morning sample, measured on two occasions when low), free or bioavailable testosterone, SHBG, and often estradiol, LH/FSH, prolactin, and PSA depending on age. Because testosterone therapy can raise red blood cell counts, a CBC with hematocrit is also important, along with baseline lipid and metabolic markers such as A1c.
What is typically measured for women?
For women, the panel depends on where you are in life. Work with a clinician who selects markers for your situation rather than a fixed list. In general, evaluation may include estradiol, progesterone, testosterone, FSH and LH, and markers that shape hormone symptoms such as thyroid function, vitamin D, iron/ferritin, cortisol, and fasting insulin. The beauty of a guided approach is that the panel is chosen for you, not from a menu.
| Marker group | Why it is checked |
|---|---|
| Testosterone (men) | Confirms low androgen state when symptoms are present |
| Estradiol | Affects bone, mood, and symptoms in both sexes |
| FSH / LH | Helps clarify where you are in reproductive aging |
| Thyroid panel (TSH, free T3/T4) | Thyroid problems mimic hormone imbalance |
| Vitamin D and iron/ferritin | Common contributors to fatigue and low energy |
| CBC / hematocrit (men on testosterone) | Monitors red blood cell changes with therapy |
| PSA (men over 40) | Prostate-related baseline before testosterone therapy |
How is low testosterone in men defined?
The Endocrine Society's 2018 guideline defines a diagnosis of hypogonadism as symptoms of testosterone deficiency plus unequivocally and consistently low fasting morning total testosterone, measured on two occasions. It recommends against starting therapy in men planning fertility in the near term, or in men with certain prostate cancers, a palpable prostate nodule, elevated PSA, hematocrit above 48 to 50 percent, untreated severe sleep apnea, severe lower urinary tract symptoms, or uncontrolled heart failure. During therapy, hematocrit and PSA are monitored.
Why symptoms and labs go together
Low energy, mood changes, and low libido can come from hormones, thyroid, sleep, stress, nutrition, or a combination. A lab value alone does not tell you what to do, and a symptom alone does not tell you what to test. The value of a physician-guided evaluation at Zest is that your history and your lab work are interpreted together, and unrelated findings are not brushed aside.
