Men's hormone therapy at Zest Regen Health starts with testing, not treatment: symptoms alone do not establish low testosterone, and current guidelines require symptoms plus confirmed low levels on repeat early-morning bloodwork. The clinic in Columbia, Maryland offers physician-guided evaluation, diagnostic testing, and individualized hormone optimization for men who qualify. Treatment decisions are made with monitoring and follow-up built in.

What symptoms lead men to seek evaluation

  • Low energy or persistent fatigue
  • Reduced libido or erectile concerns
  • Mood changes, irritability, or low motivation
  • Decreased strength, endurance, or exercise performance
  • Sleep changes or weight gain, especially around the midsection

These symptoms overlap with many other causes, including stress, sleep problems, thyroid issues, and medication effects, which is exactly why testing comes before treatment.

How low testosterone is diagnosed

The Endocrine Society's clinical practice guideline defines hypogonadism as symptoms or signs consistent with testosterone deficiency plus unequivocally and consistently low serum total or free testosterone, confirmed on repeat early-morning fasting measurements. A single random blood draw is not enough to make the diagnosis.

What testing at Zest includes

  • Total and free testosterone measured on an early-morning fasting sample
  • A repeat measurement to confirm low levels
  • Related labs such as estradiol, thyroid markers, hematocrit, and PSA where appropriate
  • A review of medications, supplements, and health history that can affect testosterone

What a personalized plan may include

  • Foundations first: sleep, nutrition, activity, and stress management
  • Treatment options discussed after evaluation, including testosterone therapy where appropriate
  • A monitoring plan that includes follow-up labs such as hematocrit and PSA, plus symptom review

What the TRAVERSE trial found about heart safety

The TRAVERSE trial, published in the New England Journal of Medicine in 2023, followed more than 5,000 men with hypogonadism and cardiovascular risk for about 33 months. Testosterone therapy was noninferior to placebo for major adverse cardiac events, meaning it did not increase the rate of heart attack, stroke, or cardiovascular death. The trial did find higher rates of atrial fibrillation, acute kidney injury, and pulmonary embolism in the testosterone group, which is why evaluation and monitoring matter.

Who may not be a candidate

Per the Endocrine Society guideline, testosterone therapy is not recommended for men planning fertility in the near term, or for men with breast or prostate cancer, a palpable prostate nodule, or a PSA above 4 ng/mL. Candidacy is always evaluated individually at the clinic.
