What Happens After Stopping a GLP-1 Medication?
Appetite signals return and weight regain is common after stopping a GLP-1. Here is what the trials show and why a maintenance plan matters.

For most people, appetite and fullness signals return after a GLP-1 medication is stopped, and weight regain is common rather than unusual. In the STEP 1 extension study, adults who stopped weekly semaglutide regained about two-thirds of the weight they had lost within the following year, and in the SURMOUNT-4 trial, adults switched from tirzepatide to placebo regained about 14% of their body weight over the next 52 weeks. That is not a reason to avoid these medications, but it is a reason to plan the ending as carefully as the beginning, under physician guidance.
What the trials show
In the STEP 1 trial, adults taking semaglutide 2.4 mg plus lifestyle intervention lost an average of 17.3% of their body weight over 68 weeks. In the one-year extension after the medication was withdrawn, participants regained an average of 11.6 percentage points of body weight, about two-thirds of what they had lost; by week 120 the average total weight loss from the start of the study was 5.6%. In SURMOUNT-4, after a 36-week lead-in with mean weight loss of about 20.9%, the group switched to placebo regained roughly 14% of body weight over the following year, while the group that stayed on tirzepatide continued to improve.
| Study | Design | What happened after stopping |
|---|---|---|
| STEP 1 extension | Semaglutide 2.4 mg weekly, one year off treatment | About two-thirds of lost weight regained on average |
| SURMOUNT-4 | Tirzepatide, 52-week withdrawal phase after lead-in | About 14% of body weight regained on average |
Why regain happens
GLP-1 medications act on appetite and fullness pathways in the gut and brain. When the medication stops, those signals move back toward your baseline. If eating, movement, sleep, and stress habits have not changed underneath, weight tends to drift back toward its prior set point. Clinicians describe this as biology responding to an unchanged environment, not a personal failure.
What a maintenance plan includes
- A planned approach to stopping, reducing, or continuing the medication, decided with the clinical team rather than abruptly
- Nutrition and eating-pattern support built in while the medication is working, not started after it stops
- Strength and activity guidance to help preserve lean mass during weight loss
- Follow-up labs, weigh-ins, and check-ins at scheduled intervals
- An honest conversation about whether a medication holiday is appropriate for you
What this means when you choose a program
A weight loss program that hands you a prescription and disappears ignores the part of treatment where most people lose their gains. A physician-led program plans for the full arc: evaluation and testing before starting, titration and side effect management while on the medication, and a defined maintenance strategy after.
